From 2015 to 2024, tractor incidents led to 1,012 deaths in the agriculture, forestry, fishing, and hunting industry, averaging 107 per year. Half the deaths resulted from rollovers or overturns, according to the U.S. Bureau of Labor Statistics.Most rollovers happen in crop and animal production. These staggering numbers help explain why the industry has the highest worker death rates — surpassing mining, transportation, and construction — year-after-year.The direct and indirect costs of a single rollover death are also shocking as they can quickly exceed $1 million. Direct costs can include death benefits, medical bills, attorney fees, regulatory fines, and insurance premium increases. Indirect costs include loss of productivity and sales, replacement and training, and brand and reputational damage.AdvertisementAdvertisementAnother harmful effect on the industry is that seven out of 10 farms close within five years of a death, according to the University of Iowa's Great Plains Center for Agricultural Health.But numbers don't tell the whole story, like how it is to suffer the nightmarish loss of a relative, friend, or coworker to a farm machinery incident. Facing such tragedy can be a long, unpredictable journey of heartbreak and pain with no clear end in sight.Image courtesy of Rim GuardOutcomes like these reinforce the crucial need to adopt proven tractor rollover hazard solutions, such as using rollover protective structures (ROPS) and seat belts, identifying job site hazards, and developing safety programs for farms. Doing these simple, inexpensive things is one of best ways to honor farmers and farm workers who sacrificed all and protect those working today and in the future.Two deaths too manyThe Washington State Fatality Assessment and Control Evaluation (WA FACE) program at the Washington State Department of Labor & Industries has investigated several deadly tractor rollovers. In the years 2015 to 2024, 17 farmers or farm workers in Washington died needlessly in tractor incidents. Of those deaths, 14 (or 82 percent) resulted from rollovers: 10 in crop production, two in animal production, and two in agriculture support services.AdvertisementAdvertisementIn one incident, a 22-year-old tractor operator died in a rollover at an apple orchard. He worked at the orchard for just over a year. He was using a low-profile utility tractor to lift and move wooden bins of apples. The tractor had pallet forks attached to a rear 3-point hitch. He drove the tractor down an uneven dirt road on a steep hill that had an 18 percent to 21 percent grade. When he tried turning at the bottom of the hill, the tractor rolled and threw him to the ground towards the rear.A 22-year-old lost his life in this rollover. Another worker saw the overturned machine and alerted a manager nearby. They ran to the tractor and found the badly injured operator face down, gasping for air. The manager called 911, but the young worker could not be saved and died at the scene.In a second case, a 55-year-old experienced tractor operator was killed in a rollover at a cherry orchard. He worked for his employer for over 20 years. He was using the tractor to move cherry bins from tree rows to a landing deck where they would be loaded onto trucks. The tractor had pallet forks attached to a 3-point hitch.He was traveling near the edge of a steep, 12-foot earthen embankment at the end of the rows. He appeared to make a turn before attempting to back into a row. As he turned, the tractor slid in soft soil, rolled over the edge, and trapped him under a rear tire. A worker nearby saw his legs sticking out from under the tire. He called 911, but the operator was already dead.This steep embankment contributed to a tractor rollover incident. (Image courtesy of the Washington State Fatality Assessment and Control Evaluation)What led to these tragedies?Both incidents involved newer tractors with factory-equipped ROPS and seat belts that were not being used. The ROPS on each tractor were in a down position even though there were no overhead obstructions present that would have required the ROPS to be down.AdvertisementAdvertisementThe tractors' steering, front-wheel traction, and stability were also compromised by them traveling with the added weight of rear 3-point hitches and pallet fork attachments on uneven, eroded dirt roads on hills and near steep embankments. Hazardous conditions like these should have alerted the operators to set the ROPS in an upright position and buckle their seat belts.As for their employers, they did not know about state safety rules requiring ROPS and seat belt use on farms nor did they follow the machine manufacturer's warnings. A lack of first-line supervision in the orchards also allowed the dangerous operation of the tractors to go on uncorrected. In the end, these incidents revealed major responsibility gaps in how the employers managed their safety programs or accident prevention programs (APP) that should have ensured the operators' safety.Lessons to live byMost rollover deaths, like the two above, can be easily prevented. Farms can be much better prepared to protect tractor operators from rollovers by taking the following steps:1) Make sure operators use ROPS and seatbelts properly. Always use factory-equipped ROPS and seatbelts on newer tractors according to OSHA and state-plan ROPS rules and manufacturer's operating standards. A ROPS system combines metal roll bars, frames, and crush proof cabs that are specially designed to limit the rollover to 90 degrees and protect the operator's body. Using ROPS is 99 percent effective in preventing serious injury and death when used with a seat belt. Without a seat belt, the operator is likely to be crushed by the tractor, roll bar, or an attached implement during a rollover. The seat belt should fit snugly to keep the operator inside the ROPS protection zone.AdvertisementAdvertisementInspect the ROPS and seat belt before each use and during regular maintenance intervals to ensure they stay in safe operating condition. Never use a tractor if the ROPS or seat belt are damaged, missing parts, or malfunctioning. Apply ROPS warning labels in the operator's cab that are written in languages operators understand best. Operating an older tractor that doesn't have rollover protection is a tragedy waiting to happen. This is especially true for tractors made before late 1976 when ROPS and seat belt requirements became federal law. Instead, contact a reputable farm equipment dealer to have them install a manufacturer-certified ROPS and seat belt retrofitting kit. This will ensure the kit is properly mounted and meets OSHA requirements. Do not attempt to make your own ROPS or seat belt — that's just asking for trouble.The National ROPS Rebate Program offers rebates to farmers in some states that cover up to 70 percent of kit, shipping, and installation costs. The program also assists farmers in locating manufacturers and dealers of certified original equipment and aftermarket retrofitting kits.3) Train tractor operators. Training should be a building block of every farm's safety program or APP, as it empowers operators with knowledge and skills to use tractors safely. Conduct recurrent training that strongly emphasizes the severe consequences, such as injury or death, of not using ROPS and seat belts, clearly explains requirements for using the protective equipment, and provides hands-on demonstrations on how to use it properly. Use presentations, videos, handouts, and interactive exercises to suit workers' different learning styles. Train all operators, from novices to experts, as experienced operators account for 80 percent of rollover deaths. Never allow an operator to use a tractor before evaluating their ability to do so safely.AdvertisementAdvertisementIf possible, enroll operators in farm heavy machinery safety workshops and training courses, such as AgSafety4U or WISHA 10 for Agriculture (currently only in Washington state). Provide refresher training when machinery or equipment is replaced, dangerous practices are seen or reported, and at a supervisor or operator's request. Translate training resources into languages that operators understand best. Keep training records on file and review them to ensure operators are current with their training.4) Perform walk-through site inspections. Keeping farm tractor operating areas safe is critical to prevent rollovers in the first place. Performing routine walk-through site inspections in such areas, jointly with supervisors and operators, can help identify hazards that make a rollover more likely to happen. Ensure farm roads are as smooth, even, and stable as possible by looking for deep ruts, potholes, large roots, and soft spots to remove. Check that roads are wide enough for tractors to maintain a safe driving distance from embankments or side slopes. Create alternate driving routes to keep tractors from travelling on steep hills and sharp curves.Provide ample lighting so operators can clearly see tractor controls, roads, signs, and stationary and moving objects during hours of darkness. Use high visibility traffic devices, such as signs, cones, chains, stakes, and barricades. These can alert drivers of speed limits, curves, bumps, slopes, embankments, intersections, overhead obstructions, and crossing points for pedestrians, vehicles, and animals. Operators' rollover risks change among different types of tractors, implement attachments, and job procedures. Performing step-by-step Job Hazard Analyses (JHA) to identify hazards and solutions can greatly reduce the chances of serious injury or death. Use standardized JHA forms to document any observed hazards and controls. Make sure information is written clearly and easily readable.AdvertisementAdvertisementFarm management should review each completed JHA form before signing it. Keep the finished forms in a place workers can easily access at any time. Remember to update the JHAs when any changes occur that introduce new hazards.6) Customize safety programs to suit specific farm needs. No two farms are the same, as workers, machinery, equipment, tools, materials, and work environments differ from place to place. This means farms should customize their safety program or APP policies, including tractor operation procedures and ROPS and seat belt use requirements, by ensuring they are written, organized, managed, and enforced in ways that best meet their unique needs.
